The Fora ReForm app is a tool to obtain a price, reform, and receive a new 5-year warranty for your Fora Form furniture. The app will help map the model and type, before sending a request for quote. (RFQ - Request for quote). On some of our older models, the design has changed over time. The app will ensure the right cover or part for your piece of furniture by using the form Identification/RFQ.
